Prepare for Collection

Ice packs must freeze completely flat
Open the kit. Set out all of the contents before you begin
Freeze both ice packs overnight, 8 hours minimum before collecting your sample
Scroll down to the calendar. Choose a GREEN shipping day

Save the Styrofoam top + brown return shipping box
Collection cup, semen preservative, patient ID label
Biohazard bag, thermal bag, 2 ice packs
Styrofoam top + brown return shipping box
Avoid ejaculation for 2 to 5 days before collection

Collect Your Sample
!
No preservative = sample cannot be tested
Wash and dry your hands
Ejaculate right into the cup
Add the semen preservative

Label Your Sample
!
No patient ID label = sample cannot be tested
Fill out fully: full name, date and time of collection
Time zone, days of abstinence, full sample collected
Place the label on the SIDE of the cup — not on the lid

Package Your Sample

Sample cup goes in SIDEWAYS
Place the labeled cup inside the orange biohazard bag
Squeeze out all the air. Seal the bag tightly
Place the biohazard bag sideways into the thermal bag

Box Your Kit
!
Return shipping label clearly visible on top of the box
Work on a flat surface
Roll the thermal bag up tightly like a cylinder. Seal it using the adhesive strip
Place both frozen ice packs on top, then the Styrofoam

Shipping Your Kit
!
Hand-deliver to a staffed FedEx. Not a drop box or third-party drop-off center
Drop off before the overnight cutoff
Ship the same day as collection
