Green days fall Monday to Thursday and account for holidays
Your semen sample must be collected and shipped on the same day
This collection kit should be used within 30 days

STEP 1
Let’s Prepare for Testing
Freeze both ice packs flat for at least 24 hours
Keep them frozen until shipping day
Save the brown box and styrofoam top for return shipping
Pick a green ship day on MailMySample.com, never Friday or weekend
Abstain from ejaculation 2 to 5 days before collection (2 days if DNA Fragmentation is ordered)

Collection Process
Wash your hands and penis
Ejaculate directly into the open cup and collect the entire sample
No condoms, lubricants, lotions, saliva, or partners
Pour the full preservative vial into the cup
Screw the lid until it “CLICKS”, then swirl to mix
Flip the cup upside down to check for leaks

Label Your Sample
Record full name, date and time of collection, time zone, abstinence, and whether the full sample was collected
Peel the label and stick it on the side of the cup, not the lid
No patient ID label means the sample cannot be tested

Packaging Your Sample
Place the labeled cup in the biohazard bag, squeeze out air, seal it
Insert the biohazard bag into the thermal bag
Position the cup on its side for proper fitment

Boxing Your Kit
Remove the adhesive strip, roll the thermal bag tightly into a cylinder, then seal
Set the rolled bag in the center of the shipping box, snug between the foam blocks
Place both frozen ice packs on top, then the styrofoam
Seal the top flap and keep the return shipping label visible

Results go to your provider or clinic, who will contact you directly
